'At Ypres on 28th September 1916 whilst a duel was taking place between our stokes mortars and mortars and minemwerfers of the enemy, one of our mortar enplacements was blown in by a minemwerfer shell burying 2nd Lieutenant Montgomery and his men. Lance Corporal Lewis was in charge of this mortar and he together with one man, continued to fire his mortar and engaged the hostile mortars for a considerable time notwithstanding the heavy hostile fire.'
