Commemorated in St George the Martyr's Anglican Church, Goodwood, Adelaide, South Australia. Inscription reads: 'To the Glory of God. In Honour of Our Lady, Blessed Michael the Archangel, Blessed George the Martyr & of all the Blessed Saints. To the dear Memory of all those who went forth from this Altar, where they offered the Holy Sacrifice of the Body and Blood of Christ, to give their lives in the Great War. This Screen is Blessed on Anzac Day 25 April 1922 being the Feast of Saint Mark the Evangelist.' Parents: William and Alice McINTOSH, 8 Grace Street, Unley, South Australia.

Embarked Alexandria to join the Mediterranean Expeditonary Force, Gallipoli, 9 May 1915.

Admitted to HS 'Neuralia', 17 July 1915 (dysentery); transferred to Malta and admitted to Intafa Hospital, 24 July 1915; transferred to Ghain Tuffiah, 9 September 1915; to England, 26 October 1915, and admitted to Beaufort War Hospital, Fishponds, Bristol, 4 November 1915; discharged to No 1 Command Depot, Parkhouse, 20 October 1916.

Taken on strength, Australian Detachment No 3, Parkhouse, 20 October 1916.

Proceeded overseas to France, 30 December 1916; taken on strength, 12th Bn, in the field, 9 January 1917.

Wounded in action, 14 April 1917 (compound fracture, left femur; multiple wounds left thigh and leg), and admitted to 3rd Field Ambulance, and transferred same day to 9th Casualty Clearing Station.

Died of wounds, 15 April 1917.
