Admitted to No 4 Stationary Hospital, Arques, 6 June 1916 (dental); rejoined 3rd Tunnelling Company, in the field, 12 July 1916.

Admitted to 50th Field Ambulance, 17 February 1917 (gonorrhoea), and transferred to 33rd Casualty Clearing Station; to 51st General Hospital, Etaples, 20 February 1917; discharged to Base Depot, 17 March 1917; rejoined unit, in the field, 29 March 1917; total period of treatment for venereal disease: 29 days.

Wounded in action, 29 April 1917 (gassed); admitted to 1/1st North Midlands Field Ambulance, and died of gas poisoning, 30 April 1917.
