Admitted to 3rd Field Ambulance, 18 September 1916 (influenza), and transferred to 1st Field Ambulance; to North Midland Casualty Clearing Station, 27 September 1916; discharged to duty, 5 October 1916; rejoined Bn, in the field, 6 October 1916.

Wounded in action, 9 April 1917 (gun shot wound, fractured right thigh), and admitted to 3rd Field Ambulance, and transferred to 3rd Casualty Clearing Station; transferred to 12th General Hospital, Rouen, 11 April 1917.

Died of wounds, 14 April 1917.
