Military Medal 'At PETILLON on 29th/20th July, 1916, Private CHADWICK displayed conspicuous bravery and devotion to duty, in that he took charge of a party and kept up the supply of ammunition and bombs to the front line. To do this he crossed "NO MAN'S LAND" several times under heavy fire. Our position on the left would have been jeopardised if it had not been for the prompt assistance given by Private CHADWICK and Private PERKINS. He also, at great personal risk, assisted Lieutenant DENSON, who was severely wounded with a message and was wounded by shrapnel breaking both arms. Before going to the Dressing Station he reported back to the sender of the message that it had been delivered. His bravery calls for special recognition.'

He led his platoon through heavy trench mortar and machine gun fire to the objective, and accounted for the whole of an enemy machine gun crew, bringing the gun back himself. He set a fine example of courage and determination to his men.'
