Military Medal 'For bravery and devotion to duty. On the 4th July, 1918, during the attack on HAMEL, Corporal COOK displayed great gallantry and initiative in using his lewis gun under heavy enemy fire. During the assault on the final position, where the enemy were in large numbers, this N.C.O. rushed forward with his lewis gun and opened fire on the enemy trench, thus enabling the other members of his platoon to rush the position with a minimum of casualties. During the whole operation Corporal COOK showed conspicuous courage and devotion to duty.'
