Military Medal 'For conspicuous gallantry and devotion to duty during a raid against the enemy defences North East of SAILLY-le-SEC, South West of ALBERT, on 4th July 1918. His Section Commander having been wounded early in the assault, Private JACOBS as assumed command and led his men to the objective. The enemy garrison were completely taken by surprise owing to the dash and determination with which the assault was made under this soldier's leadership. They soon closed with the enemy, and in the hand to hand fight which ensued with the bayonet, his courage and leadership were an inspiration to all those with him, and he materially contributed to the success of the operation.'
