Wounded in action, 25 April 1915 (gun shot wound, left shoulder); admitted to Government Hospital, Alexandria, 30 April 1915; transferred to Convalescent Camp, Mustapha, 20 May 1915. Admitted to Egyptian Government Hospital, Alexandria, 24 May 1915 (fever); transferred to No 17 General Hospital, Alexandria, 18 June 1915; to Convalescent Hospital, Helouan, 29 June 1915 (rheumatic fever). Transferred to England, 23 September 1915, and admitted to War Hospital, Reading, 5 October 1915; transferred to 2nd Western General Hospital, Manchester, 27 October 1915 (cardiac neurosis). Admitted to Weymouth Hospital, 27 April 1916. Taken on strength, No 1 Command Depot, 17 July 1916. Died of disease (general cerebral atrophy), Dykebar War Hospital, Paisley, Scotland, 2.35 am, 13 January 1917. Medical file notes: This patient has had several attacks of violence ... There is no doubt of his unsound state of mind. He is now fixed in a straight-jacket ... Remained in an acutely hallucinated state for some months, and dementia then set in and has been gradually deepening with signs of general cerebral atrophy.'
