'After being continuously on duty in POLYGON WOOD near Ypres from night of 25th/26th September, 1917 to 27th September, 1917 by his courage and determination kept his squad together, led them out from shelter at the 59th Battalion R.A.P. under heavy shell and Machine Gun fire, dressed and brought in two men who were badly wounded and got them back down the route of evacuation to the next relay he then went backwards and forwards between the two positions making in all three complete journeys, under fire the whole time.'
